Death Certificates

Death Certificates are available from the Teaneck Registrar’s Office if the event occurred in Township of Teaneck. State Statute requires that the person requesting Certified Copies of these records must know the event date, location, as well as other identifying information, and also establish proof of relationship to the record with identification.

To authorize a delegate to obtain a record on your behalf, use a Delegation of Authority Form.
Fees
- Certified copies - $25.00 for the first copy and $5.00 for each additional copies of the same record requested on the same day. There is a limit of 10 death certificates per customer transaction per day as per State policy.
- Amendments - $45.00, for correcting vital records. Corrected certified copies must be purchased separately when amendments have been approved by the State Department of Health. (by appointment only – plan accordingly)
We accept Cash or Money Order Only
NO Personal Checks, Credit or Debit cards are accepted for these services/documents.

Online through Vitalchek – please note there is a non-refundable order processing fee of $8.00 per order with Expedited Delivery Service available by selecting UPS.
