Vital Records

Certified copies of vital records can be obtained at the Teaneck Registrar’s office in-person by appointment only, online at Vitalchek, or by mail with an application (Spanish Application). You may also request your vital record at the State Department of Health. Please note, the Teaneck Registrar’s Office only has records for events that occurred in Township of Teaneck. All other records can be requested at the State.

Requests for records with Apostille seal must be directed to the State Office of Vital Statistics and Registry. View the State of New Jersey Department of Health website for more information.
